On May 1, 2006 the U.S. Department of Transportation posted a notice in the Federal Register concerning a small group of Luxfer composite cylinders that may not have been properly autofrettaged and/or hydrostatically tested by a third-party inspector, Arrowhead Industrial Services, Inc.
If your department has any of the listed cylinders in the Federal notice, you must discontinue use of the cylinders and return them to Arrowhead so that autofrettage and hydrostatic tests can be completed before their next use.
Luxfer has verified that the cylinders are safe based on thorough testing, detailed engineering analysis and data collected from many cylinders already recovered from the affected group. However, departments are legally required to comply with the DOT mandate.
